Hello, Jana,

I'm attaching a sample.
You need to provide your date of birth and place of birth. The fastest and most convenient way is to send a copy of the birth certificate(or also the marriage or baptismal certificate) of the ancestor from whom you want to start your search.

Good day, Michaela,

The guide traces all the ancestors of the initial person, i.e., her parents, her parents' parents, her grandparents' parents, etc.
If there are sources that say so) we can find the most significant events in each person's life, i.e., date and place of birth, parents, godparents, religion, name of baptizing priest, and name of midwife. Also the date and place of the wedding, the witnesses, the marrying priest. We also find the occupation and place of residence, last but not least, the date, cause and place of death.
I am enclosing a sample of the outtakes.
